Here at Renewing Hope Care Management, our goal is to help aging adults and those with disabilities maintain and even improve their quality of life. With values centered around Hope, Compassion, and Integrity; our vision is to treat each client with respect and to give hope in every situation.
Renewing Hope Care Management, LLC was founded by Christy Eashmond. Over the last 15 years, her passion has fueled her to help numerous aging adults and their families navigate through health challenges and aging issues. She always wants to give hope in even the most challenging times.
Christy Eashmond is a Registered Nurse. In addition to receiving her Bachelor of Science Degree in Nursing from the University of Southern Mississippi, she also obtained a Master of Public Health Degree from the University of Washington.
She is an Advanced Professional member of Aging Life Care Association and certified member (CMC) of the National Academy of Certified Care Managers (NACCM).
Return to topRenewing Hope Care Management, LLC came about not only because of her professional experience working as a nurse helping those with different health conditions and disabilities, but mainly because of her personal experience being a caregiver. Her dad battled with cancer and was diagnosed with Parkinson’s Disease at age 67. She saw him transform from what she always deemed as a “Superhero”, to someone that needed help with the simplest things, such as eating and brushing his teeth.
Although he struggled with the debilitating symptoms of Parkinson’s Disease, he never gave up! He always had hope! Being there through his journey motivated Christy to want to help others and their families through similar challenges.

It’s nice to have someone lighten your load and share the burden, so I am here to help navigate these challenging waters. As our loved ones begin to age, there are several challenges that may arise such as illness, diseases, injuries, loss of a spouse, inability to live alone or retirement. I would love to care for your loved one. You may live far away or may not know how to manage a loved one’s care.

We strive to help our clients maintain their independence and stay in their homes if possible or help to transition to other levels of care if needed such as assisted living or skilled nursing facilities. We also aid in finding the right caregiver or companion. We will accompany clients to doctor’s visits and medical appointments. Also, we offer medication management and conduct physical and cognitive assessments.
Return to topWhat makes an Aging Life Care Manager stand apart from the others is the commitment to stay focused on the needs of the client and family. Others may offer services for less, but what you receive cannot help but be influenced by the payment structure.
Hospital- or insurance-based case managers are employees of the health care system. They are good professionals. Their focus, however, is to address the issues of patients within the institution and to make efficient use of services for their organization.
We are based in the community and work with aging adults wherever they live. We provide continuity of care across all settings. We include the family and focus on improving quality of life for the older adult and their grown children.
Senior advisors are paid referral fees to suggest elder care facilities. As a general rule, they have a more limited sphere of knowledge and do not hold a professional degree needed to give them expertise in the 8 key areas of elder care. Only those facilities willing to pay a finder’s fee will be included in their list of suggestions. There may be a more appropriate option, but it will not be mentioned if the facility does not pay for referrals.
Because we are paid directly by you, we are independent and objective. Our focus is 100% on the needs of your family and your aging loved ones. We apply our considerable skill and years of training to your unique situation.
